Find/Book an instructor

All athletes must complete an instructor request/registration form. Athletes filling out request/registration form for the first time must also fill out the emergency medical information section. It is the athletes responsibility to keep information on all forms current

IF YOU ALREADY HAVE A TIMESLOT ON A CERTAIN DAY, THEN PLEASE FILL IN THE APPROPRIATE PASSWORD YOUR INSTRUCTOR HAS GIVEN YOU TO CONFIRM YOUR TIME SLOT WITH YOUR INSTRUCTOR. 

Cancellation Policy

A 24-hour notice cancellation is required. If cancelled less than 24-hours, a $30 fee will be charged.
